Test Method Principle
This Diagnostic Automation/ Cortez Diagnostics Inc. NNT4 ELISA test kit, antibody to T4 is coated on a microplate wells. This test is based on the principle of competitive immunoassay. T4 in the neo-natal sample competes with the added enzyme-labeled T4 for the limited binding sites on the T4 antibody. Separation of the unbound T4 from antibody-bound T4 and the subsequent measurement of the labeled fraction of the bound phase complete the test. The activity of the enzyme present on the surface of the well is quantitated by reaction with a suitable substrate to produce color.The employment of several references, made in whole blood, of known thyroxine concentration permits construction of a dose response curve (DRC-graph) of activity and concentration. An unknown specimen activity can be extrapolated from the DRC. For additional information and details please refer to the instructions for use.




Background Information
Thyroid hormones are extremely important and have diverse actions. Under-or over-production of these hormones (also known as hypo and hyperthyroidism) has potent effects. Triiodothyronine (T3) and T4 are the two main hormones produced by the thyroid gland. The production of thyroid hormones is regulated by the pituitary hormone (Thyroid Stimulating Hormone, TSH) which in turn is also regulated by the hypothalamus TSH Releasing Hormone (TRH). Determination of hypothyroidism within the first few days of birth has been recognized as the single most important diagnostic test in neonates by the American Thyroid Association. Congenital hypothyroidism is probably the single most common preventable cause of mental retardation. Diagnosis and treatment of congenital hypothyroidism within the first 1-2 months after birth appears to be necessary in order to prevent severe mental retardation. Decreased T4 concentrations in conjunction with increase concentrations of TSH in serum, is an early and sensitive indicator of diagnosis of primary hypothyroidism. This microplate enzyme immunoassay methodology provides optimum sensitivity.
